We offer a range of physical, occupational, and speech therapy services.
Our therapist help residents reach their goals using personalized treatment plans that focus on reducing pain, improving mobility, increasing independence, and reducing the risk of future injuries. Our exceptional team of therapists uses assessment and therapeutic techniques to help residents return to their highest level of function.
Following a joint replacement, our therapy team will develop a plan to help restore mobility and range of motion. Our rehab program helps residents recover quickly and safely.
With compassion and expertise, The Milton Home rehabilitation team helps residents reach their highest level of functioning and regain their independence. Using post-stroke rehab technology, we can improve outcomes and accelerate recoveries.
Cardiac Rehabilitation is designed for those recovering from cardiac conditions such as heart failure and heart attack — as well as from post-surgical and non-surgical procedures.
Our multidisciplinary team assists residents toward a successful recovery by offering:

Skilled Nursing Care
The nursing staff at The Milton Home is trained and has the expertise to administer a variety of advanced medical treatments.
Specialized services and treatments offered:
A complex wound is one that goes beyond simple wound dressing treatments. They can be either acute or chronic, and result from a variety of medical and/or surgical conditions. A complicated wound that is not able to heal properly can result in severe health problems and even precipitate further surgical intervention.
At The Milton Home, we provide a comprehensive wound treatment plan to maximize healing. Our certified wound treatment nurse follows specially designed policies and procedures to care for your complex wound needs.
We treat complex wounds and surgical conditions with a team that includes social workers, a dietitian, and a physical therapist. We utilize our talented team by offering care for conditions including:
- Complex Wound Physician
- Amputations
- Infected and draining wounds
- Pressure ulcers
- Diabetic ulcers
- Venous stasis ulcers and peripheral vascular disease
- Arterial ulcers
- Ischemic ulcers
- Traumatic and Surgical wounds
IV therapy assists residents with a variety of illnesses and conditions. IV therapy can be used to help treat infections, used for pain management to help alleviate pain, or to administer fluids to help treat or prevent dehydration.
The nurses at The Milton Home work closely with our residents to help them understand their medication treatments and work with our residents and their families for proper medication management upon their return home.
Long Term Care
The Milton Home offers a wide range of services to meet our residents’ personal care needs. We provide our long-term care residents with assistance on activities of daily living, such as bathing, dressing, and eating. We also keep our residents engaged through various activities and programs.
Hospice and Palliative Care
The Milton Home offers a comfortable and compassionate setting for residents and families experiencing end-of-life care. Our clinical team attends to the residents’ medical needs to manage symptoms and pain, so the resident can spend their remaining time in comfort.
Respite Care
We understand and recognize that being a caregiver is both a loving and difficult job. We also recognize the importance of taking care of yourself. The Milton Home is here to provide personalized care in a comfortable and secure environment, so that caregivers can enjoy a much-needed break. A respite stay is a short term stay with the expectation of returning to the previous living environment. Respite Care can be as short as 1 – 2 days and a maximum of 14 days. Guests staying with us for a respite stay will enjoy all the same services and amenities as our residents.

The Milton Home helps you take all the guess work out as you will find the comforts of home, the safety and security of 24-hour personalized care, the relief of chore-free living and the joy of new-found friendships. All in a unique, intimate setting adjacent to the St. Joseph River and Memorial Hospital and close to downtown South Bend.
